Through the models, teachers are able to coordinate classroom activities to suit the needs of the students, and to facilitate learning in a good classroom environment. Since teaching becomes voluntary for teachers, they are able to fully maximize their potential in teaching. When the teachers deliver their best, the students on the other hand receive the best. This no doubt improves the performance of the students in class.The classroom dynamics based on co-teaching models make the classroom interactions very unique. Having well defined classroom responsibilities and duties allows the teachers to specialize on particular classroom topics (Gail, E. et al. 2001). These topics can be taught intensively such that the students’ understanding about a particular topic is increased. The teachers are then able to provide support to individual students by ensuring that a whole class instructional flow is successfully maintained. A co-teaching curriculum is very instrumental in reflecting the students’ needs.By so doing, the curriculum is then implemented with the aim of addressing the compensatory, developmental, and academic skills. The student achievement is boosted when skills are improved based on how the co-teaching curriculum is implemented. Through such an approach, the students who have been involved in co-teaching have had their class achievement increased greatly. Other approaches that can be used in co-teaching include Station Teaching, Alternative Teaching, and Team Teaching.The diversity of teaching approaches offers teachers an opportunity to adopt the best and the most appropriate teaching approach based on the different and diverse student needs. This encourages proper matching of a given particular approach with the needs. The students’ achievement is increased when proper teaching approaches are adopted in teaching (Curren, R. 2006). Students in a co-teaching program are able to develop a comfortable relationship with the teachers through the well-built relationship between the two parties. The students can express themselves freely, sense harmony, and tension that may exist in a learning environment.A positive relationship can easily be established between a teacher and students in a co-teaching program. Through the relationship, misunderstandings that may arise in class are minimized (Capel, S. , and Turner, T. 2005). The problems that may arise in a class room environment are then resolved before they affect the students’ performance negatively. Co-teaching creates a cohesive classroom where the teacher and the students are able to work together towards achieving the goals of the courses taught in class. While they appreciate the strengths of what they experience during the learning process, they also recognize the weaknesses.By recognizing the weaknesses, the teachers and the students strive to turn the weaknesses into opportunities of improving learning. Furthermore, learning that integrates Individualized Ed ucation Plans can be developed. The plans are very important in ensuring that the needs of the students are well taken care of. Through co-teaching, the teachers are able to take risks . This means that new methods of teaching can be explored. This makes it easy for teachers to learn from each other, and hence they can grow as professionals. Conclusion Learning is very important in any society, whether formal or informal (Cohn, L.1976). The teaching methods in formal and informal education differ, but the two shares certain priciples. In formal education, co-techinh has bee used as one of the most effective teaching practices when it comes to assisting the students to improve their perfomance. They are specialists in their field. They have a range of pharmaceuticals to cater for the majority of conditions in the "serious" and "life threatening" categories.This project has arisen out of a need to review and improve the existing Phebra website. The task of the new look website will be to communicate the Phebra message in a simple easily readable format. As much information about the company should be placed on a single page, rather than splitting the information into separate pages. This should also make the site easier to navigate. As Phebra were previously known as PharmaLab, it could also be considered as a "rebranding" promotion. Therefore the look and feel of the new site is important, from the perspective of customer branding and effective marketing.T he existing design is based on a blue and white colour scheme. The main focus of the front page is the pull down menu which details the products on offer from the company. The site is fairly simplistic on the front page, in that the main sections include the business, products, ordering information and a contacts page. The main theme of the business is covered with the title "Life-enhancing medicines." This is quite effective. The news section on the left of the web page provides information on recent events. At the right of the home page a typical product is featured. The target audience is not obvious from the home page, even though the intended audience is hospitals. There are no links or information on current customers, even though they market to several countries. The areas Phebra cover is not easily identifiable from the home page information. The colour scheme is easy on the eye, and has a friendly outlook. The marketing message is good overall, but leaves room for improvement and could be more targeted. The information on the products however, is not easy to understand to the lay-person i.e. anyone who is not a pharmaceutical professional, and could be improved. Evaluation of existing design solution The existing design of the Phebra website is partly based on history, following the change in branding from "PharmaLab" to "Phebra". The site is well established and is attractive. The main problem with the site is that the information is quite technical and not understood by the everyday user. The site may be fine for pharmaceutical professionals, but in order for managers and the general public to find information a description of terms and products could be provided in a more user-friendly manner. They provide a useful contact us page, and the current format is suitable, as is. From a marketing perspective the text used does get to the point, but could do with a facelift. Their objective is to sell pharmaceutical products to hospitals in several countries. Therefore a simple statement about the products they sell is fine up to a point. However, in order to compete with numerous other companies in the same field, it would help to include demonstrations of products perhaps in flash movie format. Another way of gaining competitive advantage would be to include testimonials and a list of established customers. This would enhance Phebras reputation is a market leader in their field.

Ruxolitinib for Intermediate-2 Primary Myelofibrosis. Critical appraisal on the use of ruxolitinib for treatment in adult with intermediate-2 primary myelofibrosis. Introduction: Patients with primary myelofibrosis are prone to develop complicated infection due to defect in their humoral immunity. In addition, patients may develop complication such as portal hypertension, splenic infarction (which may lead to nausea, vomiting and shoulder discomfort), osteosclerosis, hypertrophic osteoarthropathy, occasionally periostitis, spinal cord compression, seizures, haemoptysis and gastrointestinal (GI) tract bleeding. (6, 7, 8, 9) In UK, Novartis holds the marketing authorisation for oral formulation. Ruxolitinib works by inhibiting Janus associated tyrosine kinase (JAK1 and JAK2) protein signalling. Ruxolitinib (Jakavi) is licensed for the treatment of disease related splenomegaly or symptoms in adult patients with primary myelofibrosis, post-polycythaemia vera myelofibrosis or post essential thrombocythaemia myelofibrosis but not recommended by NICE.(10) The major adverse drug reaction associated with Jakavi, documented in the summary of product characterisation (SPC) at incidence greater than 10% are urinary tract infection, anaemia, thrombocytopenia, neutropenia, hypercholesterolemia, dizziness, headache, increase both alanine aminotransaminase and asparte aminotransferase, bruising , bleeding and increase blood pressure. Novartis also recorded other common side effect patient experienced incidence between 1-10% was weight gain, flatulence and herpes zoster, whiles tuberculosis incidence was 1%.(3) The following clinical study, COMFORT-I and COMFORT-II trails as well as primary peered review articles Verstovsek S, Masa RA, Gotlib J, et al and Harrison C, Kiladjian JJ, Al-Ali HK, et al published in The New England Journal of Medicine (NEJM) is used to address the questions below; Evidence recommendation for or against the use of ruxolitinib in Mrs MN treatment Pharmaceutical care plan and medication optimisation for Mrs MN. Creditability and quality of evidence The clinical trial from COMFORT-I was a multicentre (USA, Canada and Australia), phase III, randomised, double blind trial (large sample size, n=309) that compared patient treatment in primary myelofibrosis with ruxolitinib (n=155) to placebo (n=154). All patients enrolled in the trial had intermediate-2 risk or high risk of myelofibrosis, a palpable spleen length of at least 5cm and was 18years or above. Patients excluded were those with an absolute neutrophil count of 1x 109/L or less, platelet count less than 100x 109/L. Incyte pharmaceutical funded this trial.(5) The COMFORT-II trial, was a multicentre (Europe with UK inclusive), phase III, randomised, open label trial that compared ruxolitinib (146) with best available therapy n=73 (hydroxycarbamide, prednisone, opoetin, lenalidomide and thalidomide). The trial was funded by Novartis pharmaceuticals. (4) The primary outcome for both trails was the proportion of patients having a spleen volume reduction of 35% or more from baseline and assessed by MRI or CT scan. The primary efficacy outcome was measured at 24 weeks in CONFORT I and 48 weeks in COMFORT II. Also the COMFORT trail (50% of primary myelofibrosis PM) population of patients with different subtypes of myelofibrosis did not reflect the global prevalence (i.e. PM is 30 times more) data reported. In addition the trials were not powered to measure overall survival or to detect statistically significant differences between subgroups (that is sex, myelofibrosis subtype), IPSS risk category or JAK2 mutation status.(1,2,3,4,5) Patient background verse evidence: Mrs MN creatinine clearance (CrCl) is 60ml/ min (normal about 100-125ml/min). The UK guideline for identification, management and referral March 2006 show that, she has stage 2 mild degree of renal function. But from SPC it is unclear, how this will increase Mrs MN risk of taking ruxolitinib. I must point out that Mrs MN is overweight with a BMI of 28 and ruxolitinib common side effect is weight gain (1-10% incidence rate). Mrs MN is capable of carrying out light house or office work from her ECOG status 1. Again patient is taking clarithromycin prescribed by GP, for possible chest infection. Novartis pharmaceutical (Javaki SPC) advises to treat any infection prior to taking ruxolitinib. (3) Mrs MN presenting complain with symptoms of anorexia, lethargy, night sweats, fever and a productive cough is a suggestive of tuberculosis (TB) infection. She is returning from holiday where risk of getting TB infection is high. If Mrs MN is prescribed ruxolitinib, she has high chances of developing complicated TB. Base on the evolution of patient background and evidences, I will not recommend ruxolitinib treatment for Mrs MN. Since Mrs MN will be receiving treatment for tuberculosis (isoniazid/ rifampicin) for at least 6 months, there is significant interaction between isoniazid and clarithromycin. Isoniazid will increase the level or effect of clarithromycin by affecting hepatic or intestinal enzyme CYP3A4 metabolism. Hence clarithromycin dose be reduce when taking with isoniazid and monitor closely. (4, 5, 11) If opting for rifampicin TB treatment, rifampicin will decrease the level or effect of clarithromycin by affecting hepatic or intestinal enzyme CYP3A4 metabolism. Hence, increase the dose of clarithromycin for the duration of treating chest infection.(11) Also, Mrs MN should be advised to recognise signs of liver disorders to discontinue treatment and seek immediate medical attention if symptoms such as vomiting, nausea, malaise and jaundice develop. However, the main question in today’s times is whether or not censorship is necessary in today’s world, and if the government should be able to force censorship on the entertainment industry.   Ã‚  Ã‚  Ã‚  Ã‚  Though the case involving the NASCAR driver is the most recent ridiculous form of censorship, it has taken place throughout the history of entertainment itself. However, in the past ten years, after censorship laws had begun to loosen, the media and the government has once again begun to pull tighter on the censorship blindfold, covering society’s eyes. After the shootings in Colorado, many wrongfully took up arms against the entertainment industry, convincing society that the blame for the teen’s actions should fall on the violence they viewed in movies and on TV. Therefore, following the shootings, the House of Representatives held a hearing to help determine any new bills that should be put into place to restrict the entertainment industry. As written by Virginia Postrel in an article for Reason magazine... ...utting blame else ware, parents, as well as other authority figures, are not solving the problem, rather, they are throwing dry wood on a fire which is already beginning to burn out of control. Censorship is not, nor has it ever been an answer. It is simply another way to cover up this modern reality, and though at times it may be harsh, it is what is real that makes one into who he or she shall become. By allowing censorship, one is hurting both those who create media art and the audience for whom the art is meant. Wart and The Master   Ã‚  Ã‚  Ã‚  Ã‚  Many people wish to be an animal if only for a day, just to see what it is like to be that animal. The obvious problem is that nobody knows how to turn himself into an animal. However, in T.H. White's Once and Future King, Wart has the opportunity to experience life as an animal because his tutor, Merlyn the magician, transforms him into many different animals. Of all the adventures, the most significant transformation to Wart's kingship occurs when he becomes a badger because the badger teaches Wart valuable lessons about human behavior.   Ã‚  Ã‚  Ã‚  Ã‚  Wart's transformation into a badger provides him with insight to become a great king. The badger teaches him certain characteristics about humans which help him to rule his kingdom. For example, he states, 'True warfare is what happens between bands of the same species'; (194). The animals in Wart's other transformations teach him only about their societies. The most important information that the badger gives to Wart is that humans are one of the only species in the world who fight among themselves. The badger supports this statement when he says, 'There are more than four thousand different sorts of them, and from all those kinds I can only think of five which are belligerent. There are the five ants, one termite that I know of, and Man';(194). This insult influences Wart to create the Round Table when he becomes the King. The purpose of the Round Table is to get all the barons to stop fighting among themselves and to form an alliance to fight only to protect th e weak. Therefore, the badger's insult influences the creation of the Round Table.   Ã‚  Ã‚  Ã‚  Ã‚  Wart's adventure as a badger was the most beneficial transformation to him as a king.
